Case Management for the Developmentally Delayed Waiver (DDW)

 

What are the Developmental Disabilities (DD) Waiver? 

It is a waiver program that helps eligible New Mexicans live in their home and community by allowing them to choose from a menu of services and supports. The person with a disability, or their representative, completes the registration paperwork and their name is placed on a computerized list of everyone awaiting services through the DD Waiver called the Central Registry.  

Who is eligible? 

To qualify for services, the individual must: 1) have a diagnosis of an intellectual developmental disability (DD) or a related condition (such as cerebral palsy, seizure disorder, autism, chromosomal disorders, syndrome disorders) that occurred before the age of 22,  2) be a resident of New Mexico.  For more details contact a Developmental Disabilities Supports Division

J&J Home Care, Inc. has four offices, Artesia, Roswell, Hobbs, and Clovis, we Case Manage individuals in 9 counties of southeast NM; Guadalupe, Quay, de Baca, Curry, Roosevelt, Lincoln, Chaves, Eddy, and Lea. We have about 14 Case Managers with about 315 individuals.  WE ARE THE LARGEST IN THIS REGION THAT ARE UNDER THE DD WAIVER.
