Case Management services include development and implementation of the Individualized Service Plan, exploring alternatives, referrals, coordination of care, facilitation of the Interdisciplinary Team, monitoring appropriateness of services to include cost-effectiveness of the ISP, and reassessment of medical eligibility. Case management is a system for insuring that a comprehensive program is implemented that will meet a recipient’s need for care and seeking to enhance the recipient’s self-care and self-determination with optimal recipient/family participation. Works with the community to provide optimum services for clients.

Must possess either a licensed Social Worker or a licensed Registered Nurse OR possess a bachelor’s degree in Social Work, Counseling, Psychology, Special Education, or closely related field.
Must have one year clinical experience, related to the target population.
Have a working knowledge of the health and social resources available. 4. DDWaiver CM must meet the requirements of training per DDSD - LTSD training & policies.

Due to being a Required Degreed Professional; Case Managers Services include but are not limited to the following:
Meets face to face with the clients as per standards according to the D.D. waiver and maintains contact by phone or home visit as frequently as needed according to the needs of each client.
Informs clients of their rights and responsibilities, grievance procedures and right to confidentiality.
Provides on-going assessments, including the collection of information from all necessary individuals that know the client. The assessments will follow the DDWaiver Standards.
Facilitate determination for the DDWaiver and other types of available community service options.
Organize and facilitate service planning process in accordance with DDWaiver Standards.
Assist in development of supplemental services.
Coordinate and monitor service delivery, including assuring services are delivered, as described in ISP and regulations, and that services are provided in a safe and healthy environment.
Develop and monitor budgets, with needed revisions or additions.
Promote self advocacy.
Maintain a current and complete Master record for the client.
Complete LOC packets as outlined in DDWaiver Standards. Standards.
Write and complete ISPs and Waiver review Forms.
Complete comprehensive individualized assessments and update as needed.
Adheres to timelines as outlined in the DDWaiver Standards. All to be overseen by trainer and direct supervisor.
Facilitating Interdisciplinary team meetings and insures follow up on realistic desired outcomes with measurable outcomes for all clients.
Follows all standards for DDWaiver.
Clerical responsibilities that are performed in an office setting, i.e knowledge of using a copier, fax machine, computer and related programs. Communicating via the internet; opening, reading and understand e-mails, inner office Memo’s. Must have current knowledge of forwarding and attaching documents via e-mail.
Case Managers are responsible for communicating effectively with their immediate Supervisors, clients, families, therapists, DDSD, DHI, Quest, Guardians, co-workers and any additional entities deemed necessary via phone, e-mail, standard mail and through Face to Face contact.
It is the responsibility of the Case Managers to keep themselves informed of continuous changes & updates within the DDWaiver Standards and J &J Homecare, Inc Policies & Procedures as deemed by the Board of Directors. This includes using the most current forms distributed by Supervisors, DDSD, DHI, Quest and ISD. i.E.(RORI’s).
All training to be overseen by DDSD Trainer, Company Trainer and Case Management Supervisor.

Employees are not authorized to release confidential and private information about clients, other employees, physicians, or others involved in client care to any person unless such person is authorized to receive information in order to carry out their duties or release of information form has been signed. Employees will follow all policies and standards as set up in the Health insurance Portability Accountability Act.

Must provide own transportation with current driver’s license, good driving record and proof of auto insurance.
Universal precautions at all times while in client home.
Dress code is per policy, to be neat, clean and appropriately attired at all times.
